Please call our office at 408.358.5086 to schedule a convenient time to meet. Patients are seen Monday through Friday by appointment. (Please indicate if you are in immediate pain or need urgent care.) Set aside 60 minutes for your first visit to allow for a thorough history, exam, and treatment. Subsequent office visits will require approximately 15 minutes for treatment.

Alternatively, you are welcome to book an appointment online at your convenience. Please be sure to let us know at least 24 hours in advance if you need to cancel or reschedule an appointment.

Our office does not accept medical insurance, we require payment for each visit at the time services are rendered. We will provide you with a receipt and an insurance claim form that you can submit to your insurance company to seek reimbursement. Our fees for an initial evaluation (ranging from 45 to 60 minutes with the doctor) are $150. Follow-up visits are billed per procedure performed and average $75 per visit. Outside lab and imaging fees are performed at our cost, with no added mark-up.

We do accept Medicare as a Non-Participating Physician. This means that we collect the full amount of your visit charges at the time of service and then send in your Medicare billing for you. Medicare and your secondary insurance policy (if you have one in place) will then reimburse you by sending a check directly to you in the mail. Medicare generally reimburses the patient 80% of the fees charged for adjustments.

The initial exam and treatment as well as supplement/orthopedic support sales are payable at the time of service. We accept cash, check, and VISA/MC.
